 Grant of Approval for Public Testing Laboratory and
Blood Storage Centre

If all conditions as prescribed by the act are complied approval is granted.

Renewal of Approval of Public Testing Laboratory is required to be done as per provision laid down under D&C Act 1940 and rules thereunder. Permission for Blood Storage Centre is to be renewed as per guidelines issued by this administration.

Application
|
|
The applicant has to make application in the Form 36 for renewal of approval for Public Testing Laboratory. In case of Approval for Blood Storage Centre application in the Form of covering letter is to be submitted. The applicant has to pay Necessary fees as given in Appendix A .
|
|
The mode of payment of fees is same as fresh approval.
|
|
Documents to be enclosed along with application of renewal of approval :-
|
- Copy of cash receipt or challan for payment of fees.
- Xerox copy of previous approval certificate. .

Xerox copy of the list of Drugs previously approved.(in case of Blood Storage Centre.)

Full particulars of the competent technical staff appointed for manufacturing and testing along with their educational qualification and experience certificates.
- Agreement between the Blood Storage Centre and the Mother Blood Bank.(in case of Blood Storage Centre.)
